What’s New at ASTRO 2016
The American Society for Radiation Oncology (ASTRO) Annual Meeting & Exhibition is the world’s largest scientific gathering for the radiation oncology community, with more than 11,000 professionals attending each year.
This year’s theme ‘Enhancing value – improving outcomes’ is essential to the future of the field of radiation oncology. With the latest technologies on display, the challenge is to discover the best ways to apply these innovations to ensure the greatest outcome.
The Annual Meeting program has been designed to address narrowing the competency gaps of patient care, medical knowledge and communication, and to promote work between interdisciplinary teams.

INDICATION SPECIFIC SOLUTIONS WITH COMPLETE WORKFLOWS
At this year’s ASTRO, Brainlab will be presenting a broad range of the Elements portfolio, focusing on Cranial, Multiple Brain Mets and Spine indications.
All Elements are designed to streamline and expedite the radiotherapy treatment planning process.
ELEMENTS CRANIAL SRS*—INHERENT INTELLIGENCE FOR CRANIAL INDICATIONS
For cranial indications, Brainlab presents Elements Cranial, a unique application that optimizes the planning process based on cranial indications—from AVM, Pituitary Adenoma, Vestibular Schwannoma, and Meningioma to large metastases.
The workflows include Distortion Correction*, a deformable co-registration with the distinct focus to compensate for distortions in MR datasets and empower high-precision contouring and Fibertracking across modalities. Fibertracking, a part of the Elements portfolio, enhances anatomical images with information about neural structures.
With the implementation of a 4π algorithm into the Cranial SRS software, Brainlab Elements are now set to become even more intelligent and efficient. 4π—licensed exclusively from Dalhousie University in Nova Scotia, Canada—enables patient tailored, optimized treatment trajectories aiming at minimizing radiation dose to organs at risk without compromising target coverage or treatment times.
Adaptive Hybrid Surgery, dedicated to multi-modality treatment of benign skull base tumors, offers radiosurgery guided surgery. The software calculates three possible SRS treatment plans and visualizes criteria to assess their effectiveness and safety, helping to balance surgical risk with radiosurgical toxicity.
ELEMENTS MULTIPLE BRAIN METS SRS—FAST TREATMENT, FOCUSED DOSE, REVOLUTIONARY TECHNOLOGY
Multiple Brain Mets SRS allows for the delivery of highly conformal single-session treatments while helping to minimize dose spread to surrounding healthy tissue. Planning and treatment time can be significantly reduced even with as many as 10 tumors to treat.
In the pre-planning section, the workflow is completed by paradigm-shifting new Anatomical Mapping*, which labels brain tissue classes without gaps. Anatomical Mapping enables robust segmentation of anatomical structures and sustainable elastic fusions that respect the anatomical environment and indication.
Brand new this year at ASTRO is TRAMs*—Treatment Response Assessment Maps—a method developed in collaboration with Sheba Medical Center in Tel Aviv.
TRAMs are used in neuro-oncological decision making to differentiate tumor progression from treatment effects such as pseudo-progression or radiation necrosis.
Brainlab will also present detailed information about the SRS Registry, a data extraction technology launched in partnership with The American Association of Neurological Surgeons (AANS) and the American Society for Radiation Oncology (ASTRO). The patient registry gathers important patient data, aiming to define national patterns of care in radiosurgery, with an eye to improving health care outcomes, supporting informed decision-making and potentially lowering the cost of care for patients.
ELEMENTS SPINE SRS*—STREAMLINED WORKFLOW TO TRANSFORM MANAGEMENT OF SPINAL TUMORS
The Spine SRS workflow presents solutions to many of the current challenges in spine radiosurgery, including registration between MR and CT in a complex region, consistent and accurate target definition and automatic creation of a highly conformal dose to avoid the critical spinal cord structure.
To address these issues, the Spine SRS workflow features Anatomical Mapping and labeling of spinal structures to enable anatomy-based deformable registrations, multimodal tumor definition, automatic CTV calculation and 3D visualization. Also included are Curvature Correction*—multi-level vertebrae matching along the spine enabling multi-modal contouring and CTV definition—and Smartbrush Spine*—a software that consistently and rapidly creates target volume by automatically applying the International Spine Consortium Guidelines for Elements Spine SRS workflow.
PATIENT POSITIONING AND MONITORING WITH EXACTRAC®
In combination with Elements, Brainlab will showcase workflows for Cranial and Spine using ExacTrac® Patient Positioning and Monitoring.
With over 850 installed systems worldwide, in-room X-Ray based ExacTrac® Positioning and Monitoring system has redefined automation, efficiency and patient positioning flexibility in the treatment room.
Instantaneous X-Ray imaging with proprietary 6D fusion provides fast and highly accurate position verification and reduces the possibility of geographical miss due to patient motion or internal anatomical shifts.
Thanks to the 4π algorithm used during planning, ExacTrac is the only system that can detect intra-fractional tumor motion even at non-coplanar couch angles.*
