Cranial 4Pi is a specialized immobilization system designed for frameless cranial radiosurgery and head & neck treatments. The system is patient specific and customizable to a broad range of anatomy. Each component is dose-optimized to allow for optimal treatment delivery and imaging.

Optimize radiosurgery immobilization

Our non-invasive, frameless system is optimized for radiosurgery and ensures precise treatment delivery by allowing for clear internal anatomy imaging. A high level of mask rigidity combined with optimized material properties support accurate dose delivery for desired treatment outcomes.

Simplify your daily routine

Every element of the design, from the lightweight overlay boards to the click-and-go masks, promotes efficient handling so therapists can quickly and securely make modifications to the setup. This is key to meeting the demands of a dynamic treatment schedule where different indications may require multiple setup changes within a single day.

Adapt seamlessly to every patient

The modular head support accommodates patients with kyphosis who cannot lie flat, providing a comfortable and stable setup. The simple height adjustment mechanism allows for precise mask fitting, which compensates for changes in facial anatomy due to edema or weight loss over the course of treatment, ensuring consistent immobilization and treatment accuracy.

“We’ve tried masks from several different providers and for us, Cranial 4Pi Immobilization is ahead of the rest. It’s clear that this system was designed for ease of use for RTTs and most importantly with patient comfort in mind—from the lightweight overlay boards and one-click fixation to thoughtful design of the masks with openings for the eyes.”

Adaptive immobilization for every case

Immobilization for cranial SRS treatments

The SRS Mask is designed with patient comfort in mind. Highly rigid immobilization makes the mask ideal for high-dose, single fraction cranial SRS treatments. This patient-specific thermoplastic mask features a three-layer design, including two anterior sheets with eye openings, a custom nose bridge, and a posterior mask, ensuring a secure and precise fit. Furthermore, the mask can be tracked seamlessly using the ExacTrac Dynamic thermal-surface camera and x-ray imaging.

Immobilization for head & neck treatments

The Extended Mask provides highly stable immobilization for cervical spine and head & neck indications. Designed to also secure the shoulders, it enhances patient stability thereby allowing for optimal positioning. The mask set features a specifically formed inlay that accommodates cervical extension, enabling unobstructed dose delivery and clear imaging throughout setup, treatment and monitoring.

Seamless compatibility

The lightweight Treatment Overlay boards are engineered to work with leading treatment table tops, including Varian® IGRT, Elekta iBEAM ®, Qfix® kVue™, and All-in-One kVue™. The CT Overlay board is compatible with the common table tops in CT simulation. These overlay boards adapt effortlessly to your existing setup while delivering reliable and precise immobilization.

Intuitive fixation and adjustment

The fixation system allows for easy mounting and removal of masks as well as quick adjustment of mask space in case of weight loss or edema. There is no need for spacers due to the simple height adjustment mechanism which allows for +/- 2 mm adjustment of the mask fitting.

Accommodation for different patient setup needs

The modular head support is designed to support patients with flexed cervical spine positions (kyphosis) who cannot lie flat on the treatment table. Combining the modular parts, like spacers and wedges, allows you to adapt the vertical position and the support angle of the patient’s head, enabling immobilization on a case-by-case basis. Additionally, the Indexing Plate allows for variable longitudinal positioning along the Headrest Platform, providing further flexibility in setup.

The SRS Mask and Extended Mask are specifically optimized for use with the modular head support set, enabling a stable and secure fit during treatment.

Additional immobilization options

The Open-Face Mask is ideal for claustrophobic patients or when enhanced visibility of the face is required for cranial indications. The mask consists of a single open-faced anterior sheet paired with a posterior mask and allows for internal anatomy imaging complemented with thermal-surface tracking.

The Basic Mask is intended for treatments requiring less rigidity, such as whole brain radiation therapy (WBRT) and palliative care. The mask features a single closed-faced anterior sheet designed for fast molding and is used in conjunction with a comfortable inlay/cushion.
